Bioidentical Hormone Replacement Therapy (BHRT) is one of the most impactful and life-improving treatments we offer. It’s a personalized, long-acting approach to hormone optimization that addresses a wide range of symptoms — physical, emotional, and cognitive — that come with hormonal decline in both men and women.

What Bioidentical Means

Bioidentical hormones are derived from plant sources but molecularly identical to your natural hormones.  This difference means your body recognizes them exactly as it would to its own hormones making them more physiologically compatible than older synthetic alternatives.

What Makes Pellets Different

Hormone replacement with pills, patches, creams, and gels can improve hormone levels, but I is difficult to optimize them with these methods.  Pellet therapy is simply superior.  Pellets are tiny, rice-sized compounds inserted under the skin in the subcutaneous fat.  They are typically placed in the backside near the hip and slowly dissolve over 3-5 months.  This consistent release eliminates peaks and valleys associated with inconsistent absorption and nearly automates hormone optimization as pellets only have to be placed 3-4 times per year. 

The Procedure

A small area near the hip is numbed with local anesthetic, a tiny incision is made, and the pellets are inserted into the subcutaneous fat. The site is then closed with a small adhesive.  The whole process takes just a few minutes and is virtually painless.

What It Addresses

Hormonal decline touches virtually every system in the body. Hormone optimization improves mood, sleep quality, depression, anxiety, mental clarity, muscle mass, libido, skin quality, hair, and bone density. Emerging research also points to meaningful long-term cardiovascular benefits and a potential reduction in Alzheimer’s risk.

For women, the most common concerns addressed are the hallmarks of perimenopause and menopause including hot flashes, night sweats, vaginal dryness, sleep disruption, weight gain, muscle loss, brain fog, mood changes, and loss of libido. 

For men, BHRT addresses the gradual testosterone decline of andropause: fatigue, decreased muscle mass, weight gain, reduced sex drive and performance, low motivation, brain fog, and mood changes that many men have simply accepted as “just getting older.”

Duration & Logistics

Pellets last 3–4 months for women and 4–5 months for men.  Follow-up bloodwork is then done just once, four to six weeks after the first insertion, to refine dosing. It’s genuinely low-maintenance.

The Optimization Process

Hormone optimization is a personalized process.  Most patients report relief/improvement within a month while full optimization could potentially taking up to six months.  Some patients, however, have been so low or so long that they report maximal improvement after their second insertion. 

Important Considerations

Pellet therapy is a medical treatment requiring proper screening, bloodwork, and follow-up. Not everyone is a candidate — patients with certain hormone-sensitive cancers or clotting disorders require careful evaluation. A thorough consultation and baseline labs are always the first step.
